PEPTIDES & METABOLIC HEALTH

A medical approach
to weight and health.

Some peptide medicines have substantial evidence for weight-related disease. Their value depends on the right diagnosis, the right product and a plan for ongoing care.

Evidence and U.S. status checked October 21, 2026. Educational topics; availability and clinical suitability require confirmation.

MORE THAN A NUMBER

Understand the goal.
Build the care plan.

Obesity is a chronic health condition. Medicines can be one part of care alongside nutrition, physical activity, sleep, behavior support and management of related illness. Treatment decisions should focus on health and sustainable function rather than promises of a particular appearance.

GLP-1 means glucagon-like peptide-1; these medicines affect pathways involved in appetite and glucose regulation. Tirzepatide also acts on GIP (glucose-dependent insulinotropic polypeptide) receptors. A treatment that helps weight or metabolic disease is not automatically a stem-cell or tissue-regenerating therapy.

WHAT LARGE HUMAN TRIALS SHOW

Meaningful results.
Specific study populations.

These original trials help explain why certain medicines are useful. They are separate studies, not a head-to-head comparison, and their averages are not individual predictions.

STEP 1 / 2021

Semaglutide

14.9% average weight reduction

At 68 weeks, versus 2.4% with placebo. The trial randomized 1,961 adults with overweight or obesity without diabetes. Both groups received lifestyle support.

Gastrointestinal symptoms were common. Trial preparation, dose, eligibility and monitoring matter when interpreting these results.

SURMOUNT-1 / 2022

Tirzepatide

15.0%–20.9% average weight reduction across studied doses

At 72 weeks, versus 3.1% with placebo. The trial randomized 2,539 adults with obesity, or overweight plus a related complication, without diabetes. Lifestyle support was included.

Gastrointestinal adverse events were common, particularly during dose escalation. Results do not predict an individual's response.

Numbers are presented in text for accessibility. The trials differed in design and follow-up; these cards do not establish that one medicine is superior for you.

WHO MAY BENEFIT?

Eligibility begins
with a clinical assessment.

For adult weight management, common eligibility criteria include BMI of at least 30, or at least 27 with a weight-related condition. Criteria differ by medicine, age and indication. BMI alone does not establish that a prescription is suitable.

A clinician also reviews weight history, previous care, current medicines, eating patterns, pregnancy plans, relevant symptoms and personal risk. Some people are better served by another medicine, behavioral support, metabolic surgery or a different clinical evaluation.

Approved benefits extend beyond weight for selected products and populations. Wegovy has a cardiovascular-risk indication in certain adults with established cardiovascular disease; its injection also has an accelerated-approval indication for metabolic dysfunction-associated steatohepatitis (MASH), an inflammatory fatty-liver disease, with specified scarring. Zepbound has a defined obstructive-sleep-apnea indication in adults with obesity. These are separate diagnoses with specific labeling.

BENEFITS NEED A RISK PLAN

Know what to expect.
Know when to call.

Common side effects

Nausea, vomiting, diarrhea, constipation and abdominal discomfort may occur. Ask how your clinician will monitor tolerability, hydration and nutrition, and when treatment should be reassessed.

Important precautions

Discuss pancreatitis, gallbladder disease, significant stomach-emptying problems, kidney vulnerability during dehydration, diabetes medicines and upcoming anesthesia or sedation. Your full product label and medical history guide the decision.

The boxed thyroid warning

Semaglutide and tirzepatide labeling includes thyroid C-cell tumor warnings based on animal findings; the human relevance is uncertain. A personal or family history of medullary thyroid carcinoma (a type of thyroid cancer), or a diagnosis of multiple endocrine neoplasia type 2 (MEN2), is a contraindication.

Pregnancy and interactions

Discuss pregnancy plans before treatment. Tirzepatide can reduce the effectiveness of oral hormonal contraception; its label calls for additional or non-oral contraception for specified periods after starting and dose increases. Ask your prescriber for instructions.

Symptoms that need prompt care

Severe or persistent abdominal pain, repeated vomiting with inability to keep fluids down, or a suspected serious reaction need medical assessment. Trouble breathing or facial or throat swelling is an emergency. Do not wait for a website inquiry reply.

PLAN BEYOND THE FIRST PRESCRIPTION

Long-term care.
Practical questions.

A workable plan includes benefits, tolerability, costs, access and what happens if treatment changes.

  1. 01

    Choose a meaningful goal.

Discuss health outcomes and daily function, not only the scale. Decide how progress will be reviewed.

  2. 02

    Protect nutrition and strength.

Ask for appropriate nutrition and activity support, adapted to your medical condition and ability.

  3. 03

    Know the full cost.

Separate consultation, medicine, laboratory testing, follow-up and supplies. Confirm coverage and refill arrangements.

  4. 04

    Discuss continuation or stopping.

Weight regain can occur after treatment stops. Make changes with the treating clinician and plan ongoing care.

APPROVED MEDICINES & COMPOUNDED VERSIONS

The name on the vial
is only the beginning.

Compounded medicines are not FDA approved. FDA's April 2026 clarification states that semaglutide and tirzepatide are no longer on the shortage list and explains restrictions on routine copies of available approved drugs. Patient-specific exceptions require meeting applicable conditions; adding B12 does not automatically make a copy appropriate.

Ask whether an approved product can meet your needs. If a compound is proposed, understand the clinical reason, exact ingredient, concentration, dispensing pharmacy and safety plan. FDA highlights dosing errors, quality and storage concerns, and problems with semaglutide salt forms.

RESEARCH SPOTLIGHT / SEPTEMBER 2026

Retatrutide: promising results, still investigational.

The phase 3 TRIUMPH-1 trial reported substantial weight reduction in 2,339 adults over 80 weeks. Its highest studied group averaged a 25.0% reduction versus 3.9% with placebo. Gastrointestinal events were common.

Sources: [73] Jastreboff et al. / New England Journal of Medicine

Positive findings do not constitute FDA approval. As of this review, FDA states retatrutide and cagrilintide cannot be used in compounding under federal law.

YOUR NEXT STEP

Start with your goals.
Leave with better questions.

An evaluation should explain the diagnosis, compare realistic options and identify an appropriate monitoring plan. A consultation does not guarantee a prescription.
